Critical There was no employee health policy on file at the time of inspection. Employees must be informed in a verifiable manner of their responsibilty to report health symptoms as they relate to foodborne disease. Inspector gave PIC a copy of an appropriate health policy to use at the time of inspection.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Quaternary ammonium sanitizing solution was observed at 450-500 ppm. Water was added during inspection to reduce concentration to 400ppm. Corrected.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Corrected During Inspection Spray bottle of degreaser was not labeled with its common name to prevent contamination. A label was written on it during the inspection. Corrected.
