[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Corrected During Inspection Observed the men's bathroom sink to be out of order and non-operational. Sign was posted indicating so and maintenance was working on fixing it at time time of inspection. A hand sink right at the front of the kitchen is being used as alternate handwashing sink for employees that are unable to wash hands in bathroom sink.
Ensure that the plumbing system is maintained in a state of good repair and that all repairs are conduct in compliance with the Ohio building code.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed an accumulation of crumbs and food dbris on the top of the dish tank. Observed several areas of the kitchen floor to have puddles due to low spots on the floor and/or a hose emptying out onto the floor. Please ensure that these areas are maintained and any spills are cleaned up at a frequency necessary to prevent the accumulation of crumbs, food debris, and water.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

The sprinkler system in the facility was tested and updated this morning prior to opening, and water was momentarily turn off but restored prior to opening. This momentary stoppage of water caused the men's bathroom sink to malfunction and maintenance was working on fixing the sink at time of inspection.
