3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed nozzles where ice is dispensed for customers growing mold/grime directly where the ice touches before going into a customers cup.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Corrected During Inspection Observed frozen meats in reach in freezer in working containers with no label to identify the meat inside.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04(J) / Single-service and single-use articles - characteristics.
Corrected During Inspection Observed styrofoam cups on the ground in pop storage area.
To prevent contamination, materials that are used to make single-service and single-use articles: (1) May not: (a) Allow the migration of deleterious substances, or (b) Impart colors, odors, or tastes to food; and (2) shall be safe and clean.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed trash can in ladies room with no lid or covering.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.2(H) / Toilet tissue - availability.
Corrected During Inspection Observed no toilet tissue available in mens bathroom.
To prevent contamination, a supply of toilet tissue shall be available at each toilet.
3717-1-06.4(A) / Repairing.
Observed 3 large cracks in floor tiles that extend from one side of the building to the other. Cracks located in back, out in customer area, and womans bathroom. Observed trash can door below hot dog rolling station unable to close. Observed chips out of several different counter top corners and wood shelving.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed floors in back where storage areas are with dirt/debri below shelving and on floor. Observed spillage on walls including behind boxes in cup storage area. Observed dirty drying rack on 3 compartment sink- either needs to be cleaned or replaced. Observed nacho cheese spillage remains below nacho cheese dispenser. Observed spillage around customer hand sink, around pop machine, and around coffee station. Observed area below hand washing sink where customers use filled with dirt/debri/food particles and unneccessary tools/misc items. Observed drawers in self service area dirty on the bottom along with shelving in this area below items. Observed both bathrooms dirty.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(F) / Drying mops.
Observed mop sitting in mop bucket and another mop sitting on floor.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

Comments
There were a lot of boxes and clutter in the back area. Store manager stated that it is the first of the month and that they just received a shipment. Facility needs cleaned on a regular schedule and store is in need of repairs and possible updating.
