Observed the manufacturer's dating information concealed with price stamp on several canned foods on display shelf.
An FSO, RFE, or manufacturer's dating information on foods may not be concealed or altered. Correct By: 23-Apr-2014
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted on door to milk on walkin cooler. Door shall close automatically and create a tight seal when closed. Door gasket may need repaired or replaced.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Correct By: 29-Apr-2014. During re-inspection: it appeared that the cooler door was being worked on. Tools were visible, the owner had gone out to get another spring.

3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Equipment food-contact surfaces or utensils are dirty including inside of ice machine on beverage unit, creamer machine, inside of cappucino machine, and beverage line area. Clean all beverage equipment daily, including areas around beverage equipment. Ice machine can not be used or placed back into service until inspected by our office due to large accumation of mold and debris.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Correct By: 23-Apr-2014
3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty including shelves, door gaskets , doors of cooling units and walking units.

Observed employee personal items stored on food prep sink. Store personal items such as phones or employee items away from food or in a designated location where food or equipment cannot be contaminated..
