Observed buildup of mildew in ice machine.
Ice is a food item and the food contact surfaces of the ice machine shall be cleaned at a frequency necessary to prevent buildup of mildew.
X / Chemical
Observed chlorine sanitizer to be at 200ppm.
Chlorine sanitizer should be between 50-100ppm to properly sanitize food contact surfaces. Too high of levels can lead to chimical contamination.

hot water adequate. Gloves available for use. Manager not exact on temperatures but proper temps for cooking, holding, cooling, and reheating are met. Proper cooling in shallow pans and reheating rapidly are used.
