Corporate office has already provided payment for the new license. Facility is inspected and approved to operate.

Within 30 days facility shall:
-Reattach/tighten the ice machine bottom chute.
- fix tiles under the hand sink across from the 3-comp sink.
- Replace corner coving @ rear hand sink.
- Walk in freezer has trays catching ice. Maintain in good repair.
- Replace weather strip on backdoor (light should not be visible)
- Seal sink in mens RR & rear hand sink to wall.
- seal around can opener holder.
- Replace handwashing sign in mens restroom.
- Replace lights above fry/tater hot holding.
- Repair bottom seals on walk in freezer & cooler doors.
- Replace front paper towel dispenser.
- Clean the dumpster area!!
- Facility must have 1 person in a managerial roll with level 2 state approved training.
- At least 1 individual at ALL times shall have state approved level 1 (food handles) training.

Facility is licensed to operate as a Commercial risk level 4.
