3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed food used to make salads is the same food that is used to make pizza. Raw sausage next to ingredients used in preparation of salads. Store sausage away from ingredients used in preparation of salads and to prevent salad toppings from contamination, use gloves when preparing pizza and salads to protect food from contamination.
Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. Correct By: 28-Mar-2014
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Inside of deep fryer and fryer basket dirty with debirs and buildup. Clean more frequently to prevent such build up.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Boxes of cheese on floor in basement walkin cooler. Store 6 inches up.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Correct By: 28-Feb-2014
3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment is not approved by a recognized testing agency including refrigerator/freezer used to store pizza toppings and frozen food items.
Replace equipment when it is not longer functioning or maintaining charactoristic qualities under normal use with commercial grade NSF equipment.
3717-1-04.8(F) / Storage - prohibitions.
Single use cups stored on floor in basement. Store all equipment utensils, or tableware 6 inches up off the floor.
Equipment, utensils, laundered linens, or single-service articles or single-use articles may not be stored in the areas specified under this rule where contamination might occur. Correct By: 28-Feb-2014
3717-1-06.4(A) / Repairing.
Floor around ice machine and pizza line in poor repair. Owner noted he plans to replace floor soon. Discussed ensuring floor is replaced with material that is durable, can withstand frequent cleaning, and is nonskid. SOme areas of floor in prep room still need repaired or maintained of cracks.
