Critical Corrected During Inspection Mop head found in handwashing sink in warewashing area, and food debris found in handwashing sink in food prep area. Mop head and food debris were removed during inspection. Ensure all designated handwashing sinks are used for no other purpose.

Critical Corrected During Inspection Food debris-covered utensils were left on food prep table. Utensils were moved to sink during inspection. Ensure all food contact surfaces of utensils are clean to sight and touch.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated bologna wasn't date marked. Bologna was volutarily discarded during inspection. Ensure all refrigerated, ready-to-eat TCS foods are appropriately date marked if being kept longer than 24 hours after opening.

Corrected During Inspection Person In Charge was unable to produce written procedures for responding to vomiting or diarrheal events. Ohio Department of Health-approved procedure given to PIC during inspection.

3717-1-06.2(E) / Handwashing signage.
Repeat No "Employees Must Wash Hands" sign found in Men's Restroom. Ensure appropriate handwashing signage is found at all handwashing signs used by employees.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Potatoes were being stored in bags on floor of food prep area. Bags were moved off of floor during inspection. Ensure all food is stored at least six inches off the floor, barring exceptions.

3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection Ice scoop was resting in ice tray behind bar. Scoop was moved during inspection. Ensure all scoops are stored in such a way to prevent contamination from handles.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available. Ensure appropriate sanitizer test kit is available at all times.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Microwave and black refrigerator had food debris in them. Ensure all non-food contact surfaces of equipment are clean to sight and touch.

3717-1-05.4(N) / Covering receptacles.
Dumpster lid wasn't closed. Ensure dumpster lids are closed when not in use.

3717-1-06.4(F) / Drying mops.
Mop was left in mop bucket. Ensure all mops are hung to air dry.

Corrected During Inspection Employee food was on shelf with consumer food. Employee food moved to its own shelf during inspection. Ensure all employee food is kept separate from consumer food in storage.
