3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Observed facility does not have employee health policy in place. Facility needs verifiable proof of employee health policy.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Observed no date mark on cut lettice. Observed grill chicken, hot dogs, and corn dogs with a prep date of 1/28/18. Ready to eat foods should be discarded within 7 days the first being when the item was prepped. Those items should have been marked to be discarded on 2/3/18.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Corrected During Inspection Observed sauces in bottles with no label indentifying what the item is. Observed liquid in container in walk in fridge with no label indicating what is inside. Observed various fried foods in freezer with no label indicating what the product is. All items that are not easily identifiable need a label indicating what the product is. Most of the time these are sauces, spices, and various meats.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04.4(B) / Cutting surfaces.
Observed all cutting boards within facility with dark scores. Cutting boards should be properly cleaned and no dark marks. If cutting boards can not be cleaned they should be resurfaced or replaced.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Observed hand sink near drive thru with faucet that does not work properly and does not turn off. Observed 3 comp sink with faucet that constantly drips and does not turn off properly. Observed dripping ice particles and ice build up on pipe in freezer. Observed drip from hoze and handle at mop sink causing leak.
A plumbing system shall be properly maintained.
3717-1-06.4(A) / Repairing.
Observed grill freezer with top missing and cutting board being used as a replacement as the top. Observed missing floor tile underneath hand washing sink in back. Observed broken ceiling vent by back door being held up with zip ties. Observed hole in walk in cooler door. Observed walk in freezer door with missing piece on outside with glue/residue left over. Walls, floors, and ceilings should remain in good repair and be smooth and easily cleanable.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed dirt/debri build up by ice machine drains. Observed splatter marks on a few different walls. Observed ceiling vents by big ice machines with dirt/debri build up. Observed hot holder containers with dark stains and built up debri on sides making it unable to be thoroughly cleaned. Observed ice build up in the grill freezer. Observed food debri on the bottom on fridge and freezer. Observed grease build up on side of reach in cooler. Facility needs to work on cleaning under equipment and in hard to reach places. If an item can not be properly cleaned it should be fixed or replaced.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
observed electro freeze machine in building not being used. Any item that is not being used or not working needs to be either replaced or removed.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

Facility needs employee health policy in place. Facility has ODH certified serv safe. Spill kit, thermometer, and test strips available. PIC answered questions in regards to food safety correctly. Minor cleaning issues, date marking will need to be monitored in the future, and repairing. Note: one pop machine was not being used during time of inspection, they use it during the busy summer months. Pest control through terminex.
