3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed several food items to be stored directly on floor of the walk-in cooler. All food items shall be stored at least six-inches off of the floor. Employees removed these items from the floor and stored them properly. Corrected.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Repeat Observed the prep sink to be propped up by tiles and is unable to be freestanding without the support.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed an accumulation of food debris and litter underneath the small two-door freezer, the fryers, the grill top pieces of equipment, and the wracks in the walk-in cooler. More regualr cleaning underneath equipment and shelving shall occur to prevent the accumulation of food debris and litter.

3717-1-06.4(A) / Repairing.
Observed the coving on the wall to be falling off and the tiles throughout the kitchen area to be broken, cracked, and/or missing.

Received a complaint on 6/3/19 concerning a customer finding a roach in their salad. Upon arrival I spoke with the General Manager, Haley, concerning this complaint. The facility uses Orkin Pest control and they are scheduled to come bi-weekly for inspections and treatment. Invoices and inspection reports are kept on record through the e-mail account and last inspection was on 5/20/19 and Orkin is supposed to come tomorrow for their scheduled inspection. Upon inspection of the kitchen, no roaches or other insects were found in the kitchen area, in prep coolers, or in the walk-in cooler. Pest controls were observed throughout the facility - good. Continue to have Orkin come and treat the facility to maintain a pest free facility.
