3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Facility was unable to provide proof of Level II ODH certificate. Please ensure that a copy of this is at the store to ensure that at least one person of managerial/supervisory status has completed Level II Food Safety training.

3717-1-03.2(M) / Wiping cloths - use limitation.
Observed in-use wiping cloths stored on the top of cutting boards and beer coolers. Wiping cloths shall be stored in clean, sanitizer water.

3717-1-04.8(E)(1) / Equipment, utensils, linens - storage.
Repeat Observed dishes used for service to be stored upright and exposed to dust, splatter, and other sources of contamination. Please protect the food contact surfaces from contamination.

3717-1-06.4(A) / Repairing.
Repeat Observed walkway into kitchen to have busted up and broken flooring. Observed missing floor panels under 3 comp sink in bar area. Observed water damaged ceiling tile in the dry storage area.
