[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Cooler in drive through temped at 61 degrees. All TCS foods were discarded. Do not use that cooler until it is serviced. Have serviced within 2 weeks.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Standing water pooling up around slushy/ ice machine. Service whichever machine is causing all of the standing water by break area. Observed a couple fryers broken down which cannot be used. Facilty must maintain the equipment in store.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Repeat Non-food contact surface(s) not cleaned at the required frequency. Hood above fryers in need of cleaning. Shelving for clean dishes by 3 compartment sink need cleaned. All equipment legs observed with significant build up of grease.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Hand sink next to mop sink constantly leaks, only way to stop leak is to turn off hot water. This is a repeat violation from a year ago. Prep sink cannot be used due to plumbing issues. Both handsink and prep sink must be reapired and maintained so employees can use them. Facility has 2 weeks to repair these plumbing fixtures.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. General cleaning need for dust buildup on ceilieng/vents, grease and food spatter on walls, floors along wall and under equipment. Must clean facility.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Condenser in walk in cooler observed with buildup of ice along with cronic leaks. Food stored under leaking condenser discarded. Do not store any food under condenser until fixed. Food was found with tent like cover with a lot water from the condenser pooled on top. Needs serviced, 2 weeks Condenser in walk in freezer also observed with a lot of ice buildup. Needs serviced, 2 weeks. Cooler door does not want to close properly, believe the gasket needs replaced. Fix cooler door so that it will stay closed.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. AC not working in facility causing employees to sweat profusley. Have AC unit servced.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Ventilation system not maintained. Fans in walk in cooler and walk in freezer have a lot of dust build up. Clean fans.

3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat No protective shielding on lights. All lights must have protective shielding in kitchen. Replace shielding.
