Critical There was no employee health policy available at the time of inspection. Employees must be informed in a verifiable manner of their responsibility to report their symptoms and illnesses as they relate to food borne disease.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Observed sliced deli meat, sliced tomatoes, chopped lettuce, open package of american cheese, and containers of chilli and beans in cold hold units without a date mark. All refrigerated, temperature controlled for safety, ready-to-eat foods must be marked with a date to indicate the day by which it must be consumed or discarded, not to exceed 7 days. All packages were dated during the inspection- corrected. (Process Review)

3717-1-05.3(E) / Sewage and other liquid waste: Conveying sewage.
Critical Observed sign instructing employees to dispose of mop water by dumping it outside in the yard instead of through the mop sink. Mop water is grey water and considered sewage and therefore must be disposed of through the sewage system. Facility shall not dispose of grey water onto the open ground.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Facility does not have anyone certified in level 2 food safety. At least one person at the facility with managerial responsibilty must be certified in level 2 food safety with an ODH certificate.

3717-1-02.4(C)(16) / PIC: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Facility does not have any written procedures for vomiting or diarrheal events. Inspector gave facility a copy of an appropriate procedure to use.

3717-1-04.1(Y) / Temperature measuring devices.
Observed the prep cooler and the deli cooler did not have thermometers in them to measure the ambient air temperature. Thermometers must be placed in the warmest part of the unit.
