Critical Repeat Corrected During Inspection Food debris was found in handwashing sink. Debris was removed from sink during inspection, and the PIC was reminded that handwashing sinks can be used for no other purpose.

Critical Corrected During Inspection Mushrooms in reach-in cooler weren't covered. Mushrooms were covered during inspection.

Critical Repeat Corrected During Inspection Ham in cooler wasn't date-marked. Ham was voluntarily discarded during inspection.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have proof of manager certification in food protection on file. Ensure a copy of certification is present at the facility for each certified employee.

3717-1-04.1(Y) / Temperature measuring devices.
Repeat No thermometer visible in reach-in cooler in dry storage area. Ensure all coolers have a working, accurate thermometer in the warmest part of the cooler.

3717-1-06.1(M) / Outer openings - protected.
Repeat Both doors to outside from dry storage had large gaps at the bottom of the door that need to be sealed. Ensure doors to outside are properly sealed to prevent intrusion from pests.

Repeat Facility had no usable test strips. Ensure appropriate test strips are available and used.
