Critical Did not see an employee illness agreement. Employees are not informed in a verifiable manner of their responsibility to report information about their health.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Observed bar handwashing sink that is being used for purposes other than handwashing. Do not store bar items in hand sink so that handsink is always useable.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed dirty knifes, ice machine, microwave,can opener and soda nozzles through out the FSO. All food contact surfaces shall be kept clean to sight and touch.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Observed raw perch and raw chicken at 49.8 degrees. TCS foods not being cold held at the proper temperature of 41 degrees or below. PIC moved foods to the walk in cooler to get back down to temperature.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Observed working containers of poisonous or toxic materials not properly labeled through out FSO. All working chemical containers need to be labeled with the common name.Add names to containers through out the FSO.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Observed knife cover made with cardboard and tape. Multiuse food contact surfaces shall be smooth and easily cleanable.PIC discarded the cover.

3717-1-03 / Food - safe, unadulterated, and honestly presented
Observed cherry tomatoes in walk-in cooler with mold on them. Food was unsafe, adulterated, or not honestly presented.

3717-1-04.5(C) / Cooking and baking equipment - cleaning frequency
Observed Alto Sham oven and Duke oven not cleaned when required. Increase cleaning frequency to prevent build up of debris.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Observed multiple food containers through out the FSO with out any labeling. All food that are not unmistakably recognizable need to be labeled.

3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Observed bugs trapped in the light shields in the kitchen.Remove trapped bugs from premise.

3717-1-04(F) / Sponges - use limitation.
Observed sponges in the dish washing area. Sponges are not allowed to be used in the FSO for cleaning. PIC discarded the sponges.

3717-1-03.2(M) / Wiping cloths - use limitation.
Observed improper use and/or maintenance of wiping cloths in multiple locations in the FSO. Store all wet wiping cloths in sanitizer buckets to prevent contamination.

3717-1-04.8(E)(2) / Clean equipment and utensils stored in a self-draining position and covered or inverted.
Observed pans wet nesting after being washed. Clean equipment and utensils not stored in a self-draining position. allow space for air drying.

3717-1-04.8(E)(1) / Equipment, utensils, linens - storage.
Observed knifes being stored between the wall and the knife board on the wall. Store Utensils in a location to prevent contamination.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Observed back prep line cooler not holding temperature. Maintenance is required before using the cooler for TCS food.

3717-1-04.2(H)(2) / Temperature measuring devices - mechanical warewashing.
Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations. Acquire test strips for ware washing machine and use on a regular schedule to ensure machine is working properly.

3717-1-05.4(N) / Covering receptacles.
observed outdoor waste receptacles not covered properly. Ensure dumpster lid is closed at all times.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items on premises. Imperial oven sitting outside of back door needs to be removed from the premise.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed wall corner cove broken. Floor and wall junctures not properly coved to have openings no larger than 1/32 inch.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed dirty floors through out the FSO. Clean floors throughout the FSO especially under racks and all wheeled equipment.

3717-1-06.4(M) / Storing maintenance tools.
Observed fly swatter stored on drink area next to clean pitchers. all maintenance tools need to be stored away from clean food items.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed vent cover missing on salad prep cooler, and dirty fan covers in walk in keg cooler and fan in drink area. Maintain ventilation system to prevent contamination.

3717-1-06.2(I)(3) / Lighting - intensity (50 FC)
Observed light intensity less than fifty foot candles (42 FC) in required front prep area. Add more lighting to food prep locations to ensure safety.
