3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed cut onion sitting directly on the cooler shelf. Place food in containers to protect from contamination. Discard potentiallycontaminated items.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.

3717-1-06.4(A) / Repairing.
Observed a hole in the wall by the mop sink, allowing entry of pests from the outside.Repair external openings to prevent entry.
The physical facilities shall be maintained in good repair.

Observed accumulation of dust on the ewalk in cooler fans. Clean fans regularly to prevent dirt and dust from blowing onto food.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

Repeat Observed items stored in the back room which are no longer used, as well as cardboard boxes and litter. Clean out the storage room to decrease clutter.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

Hot water adequate. Observed proper holding temperatures. This facility must have one employee trained in level 2 food safety (Servsafe) by March 1, 2017. Classes are offered through our office; check our website www.pchd.org for upcoming classes. This facility needs a spill kit and written procedure to use when cleaning up vomit or fecal spills.
