[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Observed that the facility did NOT have anyone with state approved level 2 food safety training. At least one person shall have a state approved level 2 food safety training.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed water building up at the bottom of the chest beer cooler underneath the window. Clean with a wetvac to prevent damage to food or equipment.

3717-1-06.1(A) / Floors, walls, and ceilings.
Observed hole in wall behind the urinal in mens restroom. Observed that the carpet behind the bar was tearing up. Maintain surfaces throughout the facility in good repair so that they are smooth and easily cleanable.

3717-1-06.4(G) / Absorbent materials on floors - use limitation.
Observed that the women's restroom had carpet going partially in. Carpet should not be in areas where water and moisture can accumulate to prevent mold.
