Critical Improper bare-hand contact with RTE foods. Observed bare hand contact with RTE foods used for salads at prep table. Contaminated foods discarded.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Food not properly protected from contamination by separation, packaging, and segregation. Observed raw sausage used for pizzas on cutting board on prep table, worker touches raw sausage and touches other containers. A lot of cross contamination on prep table.

3717-1-04.5(B)(1) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Food equipment surfaces not cleaned at required frequency. Observed ice machine in need of cleaning, can opener in need of cleaning, and "clean dishes" with debris found. All food contact surfaces must be kept clean.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Prep cooler next to pizza oven temping at 53 degrees after several temp checks. All food in prep table was either discarded or moved to different fridge. Prep Cooler in need of servicing. Do not use until replaced or fixed.

Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed many containers of food either not datemarked or old sticker was used. Make sure all ready to eat, TCS foods are datemarked.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed molded food in back of prep cooler. Make sure coolers are cleaned out regularly to discard all old food. Food was discarded.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Working containers of poisonous or toxic materials not properly labeled. Observed chem bottles not labeled with common name. All chemical bottles must have labels.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects, rodents, and other pests. Observed cockroach in bar area next to beer cooler. Took picture of cockroach. Clean facility so that there are less places for pests to hide/ food source.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed daylighting at back door in kitchen. Fix gaps so that pests cannot enter restaurant.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface(s) not cleaned at the required frequency. Observed shelving in basement cooler with a lot of debris, Coolers in need of cleaning, Hood above pizza oven in need of cleaning. Genral cleaning needed in whole kitchen facility.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Several ceiling tiles water stained or missing in kitchen. Replace tiles.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Ventilation system not maintained. Ventilation throughout facility in need of cleaning. Vents must be cleaned so that there is no buildup of debris.
