3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are dirty. Inside ice machine needs cleaned/ wiped down. Plastic containers have build -up of food debris under edges and aren't getting clean. PIC indicated these were being replaced. In the meantime, these food contact containers must be cleaned and sanitized properly.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Correct By: 15-Aug-2014

3717-1-04.1(Y) / Temperature measuring devices.
Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. Two prep units are missing thermometers.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit. Correct By: 21-Aug-2014
3717-1-06(A)(1) / Indoor areas - surface characteristics
Observed floors, walls, and/or ceilings that were not constructed of approved materials. Old floor drain that was filled in and is collecting water from moping. Please cover or bring hole up to grade to avoid collection of debris.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable; closely woven and easily cleanable carpet for carpeted areas; and nonabsorbent for areas subject to moisture.

Time in lieu of temperature is being used for pizzas. A written procedure and some form of time marker is required if this practice is going to be used. This practice falls under a Level IV license and currently, Megadeal is a Level III.
