3717-1-02.4(B)(2)(k) / PIC: Demonstration of Knowledge - Explaining correct procedures for cleaning and sanitizing
Critical PIC unable to demonstrate knowledge of cleaning and sanitizing. Did not know he could make a bleach solution for sanitizing or purchase food service sanitizer. Did not try to come up with solution to lack of sanitizer, just opted to not sanitize dishes or food contact surfaces.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Food not properly protected from contamination by separation, packaging, and segregation. Observed containers of food being stored in egregious amount standing dirty water at bottom of cooler. Make sure cooler is fixed so that it no longer leaks, and absolutely NO FOOD should be stored in dirty standing water in your coolers.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Was observed using no sanitizer for dishes and food contact surfaces. Owner stated he ran out of sani tabs and could not find more. For a week or so there was no sanitation in kitchen. Told him to use a bleach solution or a food grade sanitizer for surfaces.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed several foods not discarded after they were passed the throw out date. Make sure you are staying up on discarding foods after date has passed.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed sliced mushrooms in cooler not datemarked. All TCS ready to eat items must be datemarked.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects. Observed numerous flies in facility. Windows were open due to air conditioning being out. Fix AC or get box fans so that windows arent open for flies to come in.

3717-1-04.7(E) / Use of laundry facilities.
Improper use of laundry facilities. Wiping cloths are being washed in owners personal washer and dryer at home. Linens for the facility must be laundered either on premises, by a food service laundering company or you buy disposable wipes.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Facility in need of good cleaning. Food debris, dust and grime observed throughout facility. Clean facility.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat Observed vents throughout facility in need of cleaning. Make sure they are cleaned at a frequency that prohibits accumulation of dustt. Clean all vents.
