[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(B) / Person in charge: demonstration of knowledge.
Critical Person in charge was unable to locate ODH serv safe. Person in charge was unable to locate spill kit. Person in charge was unable to locate thermometer or test strips. Person in charge was unable to provide employee health policy. Person in charge was unable to demonstrate proper cooking temperatures or holding temperatures. Person in charge was unable to answer any questions in relation to food safety within subway or other areas. Person in charge was unable to answer any questions in regards to cooling and reheating meatballs in subway. At any given time during the day the establishment needs to have a person in charge. The person in charge is responsible for knowing and demonstrating the 16 areas of knowledge within the food code.
To prevent or correct factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection.
3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Observed facility did not have employee health policy in place.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed facility did not have one employee with ODH certified serv safe level 2 verification.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-02.4(C)(16) / Person in charge: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Observed facility did not have vomit or diarrhea spill kit available.
Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.
3717-1-03.2(D) / Food storage containers - identified with common name of food.
Observed spices in working containers not labeled in subway area.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Observed facility did not have food thermometer available.
To prevent foodborne illness, food temperature measuring devices shall be provided and readily accessible for use.
3717-1-04.4(B) / Cutting surfaces.
Observed cutting boards in subway area with scores that can not be cleaned. Cutting boards need to be able to get completely cleaned. If boards can not be cleaned they need to be resurfaced or replaced.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
3717-1-06.2(E) / Handwashing signage.
Observed both hand washing sinks in subway with no hand washing sign.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
3717-1-06.4(A) / Repairing.
Repeat Observed broken tile below reach in fridge in subway area. Observed broken tiles below 3 comp sink in subway area. Observed chunks missing out of floor matt by 3 comp sink in subway area. Observed leak in subway area where container was used to catch water. Observed out of order sign on bathroom indicating a fix is needed. Observed constant drip at faucet in bathroom. Observed countertop coming up by self service sink. Observed leak at self service sink dripping in container below sink. Observed flickering light at coke machine cooler. Observed many different ceiling tiles throughout facility with water damage. Also any item that is no longer able to be cleaned shall be repaired or replaced.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed ice build up in ice cream freezers (all of them). Observed dirt build up below shelving and around water softner in subway. Observed build up dirt/debri under 3 comp sink in subway. Observed fan being used in subway full of dust/debri. Observed cookie making sheets with stains and uncleanable. Observed bathroom with dirty toilet, sink, and floors. Observed bottom shelving in all cabinets below self service station. Observed cabinets with trash cans that need cleaned around the cans. Observed self service sink with leak and water marks/dried liquid all over cabinet. Observed crystalized cappacino caps indicating it hasnt been cleaned in a while. Observed dirty shelving where cleaning supplies sits by mop area. Observed dirt build up on floors, walls, and items in back on right by filing cabinet corner. Observed dust debri built up by the back area by the back door. Observed walk in freezer covered in ice build up on walls, floors, and product. Observed in walk in cooler onion layers on floor, food debri on floor, dust on floor, dust on fans, old drinks and debri under shelving, and shelving holding products dirty. Observed shelving in main area with dust and debri build up. This facility is in need of some serious cleaning. The facility should be cleaned as often as necessary to keep it clean. Facility needs to clean hard to reach places, under equipment, between equipment, shelving, and areas that need to be cleaned. All things that can no longer be cleaned need to be fixed or replaced.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(F) / Drying mops.
Observed mops sitting in mop bucket. When not in use mops need to be air dried.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed broken not used oven in subway. Observed product in room in 3 comp sink area on shelving which per discussion with employee items are never used. Observed old juices and dented cans mixed in with cleaning supplies next to mop sink. This area needs to be cleaned and organized. Observed garbage on floor and old papers next to filing cabinet in back. This area needs to be cleaned and organized. Observed clutter and garbage on floor in back by the back door. This area needs to be organzied and cleaned. Observed empty boxes in walk in freezer on floor and on shelves. This area needs organized and cleaned. Observed old drinks and garbage under shelving and old empty boxes in floor in walk in cooler. This room needs to be organized and cleaned. All unnecessary items, litter, garbage, and items not being used need to be removed from facility.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

PIC unable to answer questions in regards to food safety and reheating and cooling items. PIC unable to answer questions in regards to foodborne illness. PIC did not demonstrate proper food safety or able to answer questions in regards to the 16 areas of knowledge within the food code. Facility is in need of some serious cleaning with some major issues. Facility is in need of repairs. Subway does a great job with date marking and labeling. Recommended that facility shut down voluntarily for a few days to clean and repair. For ODH serv safe level 2 classes available go to www.pchd.org under environmental tab under food safety. Facility was given examples of employee health policy, and vomit/diarrhea spill kit.
