3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Critical The plumbing system was not properly repaired. Observed water on floor in dishwash area everytime dishmachine would empty.

3717-1-06.4(K) / Controlling pests.
Critical Observed the presence of live insects, rodents, and other pests. Observed dead and live insects in the facility. Operator has contracted with a licensed pest control company,
The presence of insects, rodents, and other pests shall be controlled by routinely inspecting incoming shipments of food and supplies, and routinely inspecting the premises for evidence of pests, using methods such as trapping devices or pesticides, and eliminating harborage conditions. 3717-1-03.2(M) / Wiping cloths - use limitation.
Observed improper use and/or maintenance of wiping cloths. Observed dirty cloths in buckets under a sink in the prep area.
Cloths that are in use for wiping food spills from tableware and carry-out containers shall be maintained dry and used for no other purpose; cloths in use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer, or laundered daily; cloths used with raw animal foods shall be kept separate from other cloths, dry cloths and solutions shall be free of food debris and visible soil; containers of sanitizer solutions shall be stored off the floor, and used in a manner that prevents contamination. Single-service articles, single-use articles or single-use disposable sanitizer wipes shall be used in accordance with EPA - approved manufacturer's label use instructions. Correct By: 19-Sep-2014

TCS foods were not properly thawed. Observed a bag of shrimp thawing in the prep sink.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Fixed equipment did not appear to be properly sealed or spaced for cleaning. Observed the hand wash sink in dishwash area was not sealed to wall.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule. 3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. Observed dirt & debris on the floors in hard to reach areas. All equipment needs to be pulled away from wall and the floors needs to be cleaned.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Correct By: 19-Sep-2014

Observed a build-up of dirt and debris. Observed dirt and debris in the floor sinks that all indirect plumbing connections go to. The build up of dirt and food debris provide a food source for rodents and insects.
