[?] A summary of the violations found during the inspection are listed below.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the can opener and the mixer were not cleaned after use. Clean all equipment after using it.

3717-1-03.4(C) / Thawing - temperature and time control.
Corrected During Inspection TCS foods not properly thawed. Observed chicken wings thawing at room temperature. Frozen TCS food must be thawed properly, and since there is not prep sink at this facility, it must be thawed under refrigeration.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Food not protected from contamination. Observed a stainless tumber being used as an ice scoop behind the bar. Use a proper ice scoop to prevent contamination of the ice which is being used for drinks.

3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection In-use utensils improperly stored. Observed a knife sitting in an insert of water at room temperature with some food debris in it. Store all in-use untensils in a hygenic manner.

Corrected During Inspection In-use utensils improperly stored. Observed pizza peels stored on the top of the pizza oven and the top of the pizza oven is not being cleaned as necessary to prevent contamination of clean utensils.

3717-1-06.2(J) / Ventilation - mechanical.
Ventilation not sufficient. Observed two electric fryers without proper ventilation. Equipment that produces grease laden vapors requires a hood approved by the fire department.

3701-21-03 / Facility layout and equipment specifications for FSO
FSO did not submit facility layout and equipment specifications. Observed two electric fryers being used by the FSO that were not approved by the licensor. All new equipment not previously approved must be approved by the licensor prior to installation and use.
