3717-1-04.5(B)(1) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Repeat Food equipment surfaces not cleaned at required frequency. Observed ice machine with build up of debris. Clean ice machine.

3717-1-04.7(E) / Use of laundry facilities.
Improper use of laundry facilities. Wiping cloths are laundered a residential washer in dryer. Code states linens for food service operations must be done by a company that specializes in restaurant linens or in a washer and dryer specifically for the facility.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Ventilation system not maintained. Observed fans in walk in with build up of dust. Clean fans.

3717-1-20(B) / Specifications for replacing equipment and facilities in existing facilities
Existing physical facilities and equipment constitute a public health hazard. Facility has no handsink at the bar. Code states that there must be a handsink anywhere that there is warewashing. Possible suggestion is to replace the three compartment sink with a four compartment bar sink. Facility has 30 days to have a plan in place.
