Facility failed to produce an employee health policy. An employee health policy showing that employees have been informed of their responsibility to report any symptoms or illnesses relating to food borne diseases must be available.

IV / Demonstration of Knowledge
Facility needs at least one employee certified in level 2 food safety as shown by certificate from the Ohio Dept of Health. Facility only had Servsafe certificates.
