3717-1-03.6(A) / Discarding/reconditioning unsafe, adulterated, or not honestly presented food
Critical Corrected During Inspection Observed food left in freezer labelled as, "Keep Frozen" that were not frozen, leaking onto the outer package, and starting to mold. Employee removed them from the freezer and threw them away. Corrected.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection Observed roast beef and chicken breasts at 49F and tuna salad at 52F in the 2-door reach-in cooler, which had been preped about 1.5 hours ago. Employee moved them to the counter 2-door reach-in cooler and temperatures began to drop. Corrected.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have at least one person with an ODH level two certification in food protection.

3717-1-06.1(M) / Outer openings - protected.
Repeat Observed bottom of back door to be rusted and rotted away exposing an outer opening.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed 2-door reach-in cooler to be holding at 45F and not keeping TCS foods at the proper cold holding temperatue. Observed walk-in freezer to be completely off and not working due to an issue with massive ice build up.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed base of tomato slicer on prep table to have an accumulation of food debris. Observed an accumulation of dust build up on the condenser fans in walk-in cooler. Observed the shelving units in the walk-in freezer to have a build up of mold. These must be cleaned prior to using them again.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Observed dump sink in coffee self-serve area to have a leak with a bucket underneath collecting water. Bucket was almost full of water.

3717-1-06.4(A) / Repairing.
Repeat Observed broken floor tiles throughout the facility, observed cracked and broken grout/tile around the mop sink, observed ceiling tiles with water damage throughout the facility. Ceiling tiles have been ordered and are in storage but have yet to be installed - good.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed coffee beans and dust accumulation behind the equipment in the coffee self-serve area. Observed inside of cabinets (especially underneath the dump sink) to have stains and splatter and litter and food debirs accumulation.

Please be sure when equipment goes down, all food in the equipment is either discarded or moved to a functioning piece of equipment. Food should not sit in the malfunctioning equipment until the equipment is serviced. The 2-door cooler had issues last time, please have this cooler serviced to ensure that it is functioning properly, otherwise the cooler cannot be used to hold TCS foods (lunch-meets, chicken, tomatoes, dressings, etc.).
