Observed a crack in the floor near the dish tank which is accumulating water. Repair crack to ensure surfaces are smooth and easily cleanable, and to prevent growth of mold/mildew.
The physical facilities shall be maintained in good repair.

Overall clean and organized. Hot water adequate. Observed proper date marking and hot/cold holding temperatures of TCS foods. Ensure to obtain a sticker-thermometer for testing the hot water sanitation in the dish tank to comply with the update to the Ohio Uniform Food Safety Code.
