[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Repeat Handwashing sink water below 100°F.Hand washing sink in kitchen does not have hot water. Sink needs to be repaired to provide hot water so hands can be washed properly.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the handwashing sink(s). Observed no paper towels at hand sink in pizza dough area. All hand sinks need to be stocked with soap and hand drying devices. Refill paper towel holder so employees can properly wash hands.

3717-1-06.2(E) / Handwashing signage.
Handwashing sign(s) not posted in womens bathroom. All handwashing sinks need a sign or poster to notify employees to wash their hands. Needs to be clearly visible to food employees. Replace the sign in the womens bathroom.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed food in multiple locations throughout the FSO stored directly on the floor. All food needs to be stored at least 6 inches off of the ground. Move all food to prevent contamination.

3717-1-04.8(F) / Storage - prohibitions - single-use/single-service articles.
Observed multiple boxes in multiple locations through out FSO where boxes of to go containers and paper products are stored directly on the floor. All single use articles need to be 6 inches off of the floor. Move boxes to prevent contamination.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed dirty fridge, pizza sauce splattered on the inside of the fridge and handles with dirt build up. Clean fridge and fridge handles till clean to sight and touch.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired.Observed new dish washing machine leaking on to the floor in front of the 3 compartment sink. All plumbing needs to be maintained and in good repair. PIC states maintenance has been called for the dishwasher to be fixed

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed large collection of random, broken or old pieces of equipment, building materials and clutter throughout the basment. All unnecessary items and litter needs to be removed from the premise of the FSO.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed missing/ broken ceiling tiles above freezer in back room. All ceiling tiles need to be smooth and easily cleanable. Replace tiles that are broken or water stained.
