Critical Corrected During Inspection Did not observe any form of an employee illness agreement. License holders shall require food employees and conditional employees to report to the person in charge information about their health as it relates to diseases that are transmissible through food.
Print copies of the employee illness agreement sent through email, have each employee sign it, and keep it on file.

Critical Corrected During Inspection Observed food bags piled in front of hand washing sink. All hand washing sinks shall remain clutter-free and be accessible for use by employees.
Keep entire sink as well as the area in front of hand washing sink clear.
3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection Observed TCS foods, including freshly made pizzas, not being cold held at the proper temperature in refrigerator in back right corner of restaurant. After several attempts to measure temperature with multiple thermometers, the lowest recorded temperature was forty-two degrees. Advised TCS food be moved to another refrigerator properly cold holding at forty-one or below. TCS foods must be cold held at forty-one degrees or below.
Adjust refrigerator temperature so that TCS food is held at forty-one degrees or below.

Corrected During Inspection Observed no written procedures for responding to vomiting or diarrheal events. Food license holders must maintain written procedures for responding to illness incidents.

3717-1-04.1(Y) / Temperature measuring devices.
Corrected During Inspection Observed operating refrigerator missing a thermometer. In a mechanically refrigerated unit, the sensor of a temperature measuring device shall be located in the warmest part to measure the air temperature.
Place a thermometer in the warmest part of the mechanically refrigerated unit.
3717-1-05.4(N) / Covering receptacles.
Corrected During Inspection Observed outdoor waste receptacles lid not closed properly. Dumpster lids must be closed when not in use.
Keep outdoor waste receptacle lid closed properly when not in use.
3717-1-06.4(F) / Drying mops.
Corrected During Inspection Observed wet mop not properly hanging to dry. Mops shall be placed in a position that allows them to air-dry without soiling wall, equipment, or supplies.
