[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat observed no date marking on any food items in the fridge.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical observed build up on cooler tracks with food debri that appeared to have been there for a while.Observed thick build up layer of food particles within microwave that appeared to have been there a while. Observed sauce containers with no lids in bucket with food contact surface exposed to debri at bottom of bucket including dead bugs. observed what looks like to be either mouse droppings or dead bugs in "clean" utensil bin and food contact surfaces of utensils touching other non food contact surfaces.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
observed cut green pepper exposed in fridge with no protection. Also observed lettuce in open container turning brown color. Also observed pepperoni being stored in an open container in the fridge.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-04.5(A)(2) / Cleanliness of food-contact surfaces of cooking equipment and pans.
observed fryers with caked on grease apparent not cleaned in a while .
To prevent contamination, the food-contact surfaces of cooking equipment and pans shall be kept clean.
3717-1-04.7(B) / Frequency of laundering - specifications.
observed food and dry towels stored in same container.
Linens, cloths, and cloth gloves shall be laundered as specified in this rule.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed plates stored food contact surface up.
Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule.
3717-1-05.4(H) / Toilet room receptacle - covered.
observed ladies room with no lid on trash can.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.4(A) / Repairing.
Repeat observed handle on cooler still broken.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat observed fryers with caked on grease apparent that it has not been cleaned for a long time. observed pizza crumb built up under where pizza pans are kept. Observed dough making mixer dirty with no orders being made. Establishment had no eating guests and upon discussing with staff there had been no food sold since the night before and observed an unclean facility.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
observed trash cans outside with lids not on and garbage on ground.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

very unclean kitchen. Food contact surfaces in almost entire kitchen were dirty. Exposed food left unprotected in fridge. Containers in fridge had no date makings. A lot of equipment in kitchen is not commercial grade. Garbage cans outside were full, open, and garbage on the ground. Good fridge temperature they did have test strips, 3 comp sink, soap, paper towels, and hand washing sink with a label. Overall, the facility is very unclean and had been for a while since employee said that no one in the facility had even ordered food that day yet.
