Facility does not have employee manager certification in food protection on site.
Certificate shall be available to verify certification is up to date.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed cooler sliding door broken. Door is being held closed with a butter knife. Broken door has been mentioned in previous reports from last year. Please replace or repair cooler.
