3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Observed facility does not have employee health policy in place.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.
3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed pre made pizzas sitting in fridge not protected from contamination.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection observed cooked chicken strips with no date marking indicating when the chicken was prepped. Observed made subs that included (ham, salami, pepperoni) with a date indicating it was made on 2/1. A product that is open, cut, prepped, cooked, or made needs a date marking when the oldest item was preppared and the item is good for 7 days. Day 1 starts the day of the alteration. (cut, prepped etc)
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Corrected During Inspection Observed several sauces and spices with no label of what the item is. Any item that is removed from its original container (mostly sauces, spices, and various meats) and is not easily identifiable needs a label indicating what that item is.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04.4(B) / Cutting surfaces.
Observed cutting board at prep area with minor dark scores. Observed cutting board in back by slicer with dark scores. If cutting boards can not be cleaned completely/white then they need to be replaced or resurfaced.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Observed single service items that were previously holding peppers,sauces etc being re used to hold items. Stash of single use items located below dough maker. Single use items can not be re used.
Single-service and single-use articles may not be reused.
3717-1-05.4(F)(1) / Outside receptacles - tight fitting lids
Observed outdoor dumpster with lid open.
Receptacles and waste handling units for refuse, recyclables, or returnables used outside with materials containing food residue, shall be designed and constructed to have tight-fitting lids, doors, or covers.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed womans room with no covered trash cans.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.4(A) / Repairing.
Observed corners of wall ripped off exposing dry wood and metal. Observed busted floor air vent by front door. Observed womans bathroom with cracked and chipped floors and side panels missing on sides of stalls. Observed broken floor tiles behind bar. Observed exposed wood on floor behind bar. Observed build up of debri on exposed wood and in between cracks behind bar due to unable to properly clean. All floors, walls, and ceilings should be smooth and easily cleanable. Observed side of counter top coming off behind bar. Observed back door with broken screen. Observed water damaged ceiling tiles in back kitchen area. If something is leaking it needs fixed and water damaged items need to be fixed/replaced. Any item including floors, walls, ceilings, equipment, small holes in walls that is no longer able to be cleaned needs replaced or fixed
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed microwave in kitchen area dirty and employee states not used today. Observed splatter sauce marks on floor by prep table. Observed container holding ranch packets and other various condiments with dirt build up on the outside. Observed dirty tub and dirty dishes in tub in back by pizza maker, employee states no pizzas were made today. Observed pizza maker with crumb build up on the bottom. Observed bottom of freezers and fridges with food debri on the bottom. Observed back door with black build up on door. Observed fryer with extreme caked on grease all around inside the fryer area. Fryer should be thoroughly cleaned and all caked on areas removed. (observed smoking in fryer-potential fire hazard). Observed dough machine in back dirty which employee states was not used today. observed slicer in back with left over food debri, employee states that he has personally not used it since last week. Back of facility should be thoroughly cleaned. Clean under equipment, in between equipment, floors, walls, and ceilings. Anything that is not easily cleaned needs to be fixed or replaced.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed basement with items not being used and used as storage for items. Items that are not being used, broken, or litter needs to be removed from the premise.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.
