3717-1-02.4(C)(14) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical There is no employee health policy on file at the time of inspection. The employees must be informed in a verifiable manner of their responsibility to report their symptoms and illnesses as it relates to foodborne diseases. Inspector gave management a copy of an appropriate policy to use.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical Corrected During Inspection Observed TCS foods, such as rice and carmelized onions, that were in the hot hold unit holding at 118F. Hot held TCS items must be held at 135F or above. All items in the unit were discarded at the time of inspection.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Observed refrigerated, ready to eat, TCS pizza toppings such as lettuce and ham that were in the pizza cooler and were not date marked. Management date marked items during the inspection.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Facility does not have a person with level two certification from the Ohio Department of Health. Our office offers this class throughout the year. Go to www.pchd.org under Environmental-> Food Safety for class dates and the application.

3717-1-02.4(C)(16) / PIC: duties - ensure the facility has written procedures for vomiting and diarrheal events.
There is no written procedure for cleaning up vomit/diarrheal accidents. Inspector gave facility a copy of an appropriate procedure to use.

3717-1-03.4(C) / Thawing - temperature and time control.
Observed packaged salmon that had been thawed without removing the fish from the reduced oxygen environment. When thawing fish, you must pierce the packaging or otherwise remove it from the reduced oxygen environment to prevent growth of pathogenic microorganisms.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Observed containers of flour and sugar in large tubs that were not labeled with the common name of the item. Once removed from their original packaging, items must be labeled with the common name.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Test strips for the sanitizer are expired. New test strips must be obtained.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed some non food contact surfaces of equipment, such as the shelves next to the 3 comp sink, the bins with clean utensils in them, and parts of the ice machine, that had a build up or accumulation of residue and debris. Equipment shall be cleaned as often as necessary to prevent buid up.

3717-1-06.4(A) / Repairing.
Observed broken or missing floor tiles throughout the facility. Also observed some areas where the grout between the tiles has deteriorated. Floors shall be maintained in a good condition to allow for easy cleaning.

3717-1-06.3 (C)(1) / Employee accommodations - designated areas
Corrected During Inspection Observed an employee drink without a lid stored on a shelf above single use items in the server area. Management removed the drink at the time of inspection.
