Physical facilities not maintained in good repair. Observed significant leaking from ceiling in doorways of dry storage room, walk in cooler and chicken cooler. Leak must be fixed by 1/16/24. In the mean time, the following is to be done: Food must be stored away from doors and high on shelving units. Food must be covered when coming in and out of coolers. Water pooled in the coolers must be cleaned frequently.
