[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed ice build up in the walk in freezer. This is likely due to warm, humid air entering the cooler and freezing. The seal on the bottom of the door is broken/peeling. Repair seal on freezer door and ensure that ice build up does not cause damage to the foods within.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed an ice oar stored on top of the ice machine at the time of inspection. As the top of the ice machine is tall and wide it can collect dust which is unseen. Store the oar on the holding rack on the side of the ice machine so that it can be maintained clean and free of dust.
Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Observed that the sink at the front of the facility needed to be turned on at the mixing valve under the sink. Repair the sink fixture so that it can be easily turned on and off to promote proper handwashing and prevent any sources of contamination.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Observed bags of trash around the dumpster enclosure. Clean the dumpster enclosure to prevent attracting any pests or rodents.
A storage area and enclosure for refuse, recyclables, or returnables shall be maintained clean and free of unnecessary items.
3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat Observed that light bulbs in the dry storage area that were not properly covered. Properly shield lights to prevent contamination in case of glass breaking.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed a broken tile underneath the hand sink in the back of the kitchen. Repair tiles so that surfaces are smooth and easily cleanable.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Repeat Observed that the facility was on its last roll of paper towels at the rear hand washing sink. The DM intends to make a run for paper towels to last until next delivery day. Keep hand washing sinks properly stocked with paper towels or another method of drying hands at all times.
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed that the facility had debris building up along floor drains and behind equipment. Clean thoroughly and often to prevent food debris build up.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
