[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A) / Person in charge: assignment of responsibility.
Critical While owner was away from the FSO, PIC was not knowledgeable on food safety and prevention of food contamination from injurd employee. Lisence holder did not ensure person in charge was knowledgeable in food safety and prevention of foodborne illness.
To minimize the risk of contamination, the license holder shall be the person responsible for the FSO or RFE, or may be the person in charge, or shall designate a person in charge and shall ensure that a person in charge with applicable knowledge is present during all hours of operation.
3717-1-02.4(B) / Person in charge: demonstration of knowledge.
Critical PIC during incident was not knowledgeable on food safety and allowed employee to continue handling food while possibly contaminating food.
To prevent or correct factors that may cause foodborne illess, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Critical Repeat Observed no towels or hand drying device at the handwashing sink in the bathroom.
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Bathroom needs cleaned more frequently.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Correct By: 17-Dec-2014

Received a complaint stating Subway employee had cut on hand while making sandwiches and was dripping blood. Employee was wearing a glove, however was bleeding so much that it dripped out the bottom. Customer reported that the employee possibly needed stiches due to severity of the cut, and refused service. Upon inspection, manager reported he was not present during the incident. The employee ended his shift early. Managers on duty need to be aware of proper hygienic practices and prevention of food contamination due to cuts and other sources of contamination. If license holder is away, must ensure PIC is knowledgeable on food safety.
