Critical Food not properly protected from contamination by separation, packaging, and segregation. Observed several containers of food uncovered in kitchen. All food is to have a covering to prevent contamination.

3717-1-04.5(B)(1) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Corrected During Inspection Food equipment surfaces not cleaned at required frequency. Observed two blenders with build up of mold. All food contact surfaces must be cleaned and sanitized. Blenders were removed for cleaning.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical Corrected During Inspection TCS foods not being hot held at the proper temperature. Observed ric being held at 108 degrees. All hot holding is to be 135 degrees or higher. Rice was discarded.

Critical Corrected During Inspection TCS foods not being hot held at the proper temperature. Observed queso holding at 115 degrees. Queso was discarded.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed food in prep cooler temping at 47 degrees. All cold holding is to be held at 41 degrees. All TCS foods were disgarded. Cooler not to be used until fixed or replaced.

Critical Presence of live insects, rodents, and other pests. Observed mouse droppings on outlet by handsink. Facility has contact with a pest control company. Clean up all mouse droppings.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Repeat Food contact surfaces not easily cleanable. Observed several plastice containers holding food that were chipped or broken. All food contact surfaces must be smooth, durable and easily cleanable. Discard all broken containers.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed table under dishtank with the shlf rusted through with large hole in the middle. All surfaces in facility must be smooth, durable and easily cleanable. Replace table.

Observed employee food stored with food for the restaurant. All employee food is to be kept seperate.
