3717-1-02.1(A) / Employee health: Reporting by food employees
Critical Corrected During Inspection Did not observe any form of an employee illness agreement. License holders must require employee(s) to report symptoms, diagnosed illness by a health care provider, or high risk situations as specified in rule. Print employee illness agreement sent through email, print a copy for each employee, have him or her sign it, and keep it on file.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical Corrected During Inspection Observed bacon not being hot held at the proper temperature. The bacon was in a warming container, and it was reading at a temperature of ninety-four degrees. Multiple thermometers were used. Time/temperature controlled for safety food shall be maintained at one hundred thirty-five degrees or above, or forty-one degrees or below. Ensure all TCS foods are hot held and cold held at proper temperatures. The bacon was voluntarily discarded.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed dirty vent in walk-in cooler. Ventilation systems must remain clean and maintained. Clean vent so that it does not contaminate food.

3717-1-06.1(I) / Light bulbs - protective shielding.
Observed multiple lights in food preparation area that were not covered by protective barriers. Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food. Install light shields in the food preparation area. .
