[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(11) / PIC: duties - ensure employees are proplery sanitizing cleaned equipment and utensils
Critical Observed that the facility did not have any food contact surface sanitizer. The PIC shall ensure that employees are properly sanitizing utensils, including making sure they have necessary chemicals.

3717-1-04.5(B)(1) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Observed that the pizza and hot dog tongs were not clean at the time of inspection. Ensure that all food contact surfaces are cleaned every 4 hours. .

3717-1-06.4(K) / Controlling pests.
Critical Observed areas in the back storage area with mouse droppings. Contact a pest control operator. .

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed the gasket of the walk in cooler door hanging off. Replace or repair. Observed ice in the standing reach in cooler/freezer combo. Ensure that equipment is in good repair to prevent ice build up on food. .

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Observed damaged wood underneath soda machine and around coffee area. All non food contact surfaces should be smooth and easily cleanable. .

3717-1-04.2(J) / Cleaning and Sanitizers - Availability
Cleaning agents and/or sanitizers not available in the FSO or RFE.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Repeat Observed trash build up around the dumpster area. Clean up and maintain this area.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed a large buildup of debris in various areas throughout the store. Including but not limited to: -Rear store room -Underneath shelves in the front of the store -Underneath the shelves in the walkin cooler -Light fixtures -Underneath shelves in coffee area -Underneath ice machine -etc Clean thoroughly and often to prevent dust buildup.

PIC- Finley
Went over the violations with the PIC at the time of inspection. Discussed food safety and expectations for the follow up inspection in two weeks.
Observed rodent droppings in the rear of the facility. At the time of inspection, there was not any damaged food products. Facilities shall contact pest control operator to control the rodent issue. If any food products are found to be damaged then they should be discarded.
