[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(10) / PIC: duties - ensure employees are properly sanitizing equipment and utensils
Critical Observed that the PIC at the facility could not properly describe the process of wash, rinse, and sanitize. At the time of inspection the inspector went over the proper process with the PIC. The facility must properly train its staff to wash, rinse, and sanitize it's utensils.

3717-1-04.5(B) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Observed that the facility was not cleaning its sauce bowls every 4 hours. Any food contact surface must be properly cleaned and sanitized at least once every 4 hours. Discussed a different method of cleaning with the PIC.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Observed two bags of diced ham that were not properly dated. During the inspection these food items were dated by the PIC. Facility shall ensure that all ready to eat foods requiring refrigeration are properly dated.

3717-1-04.8(G)(2) / Kitchenware and tableware - handling.
Observed single use forks at the sub prep table that were not properly set up at the time of inspection. During the inspection the PIC moved the utensils. STORE ALL UTENSILS WITH THEIR HANDLES FACING OUT TO PREVENT HAND FROM TOUCHING THE MOUTH CONTACT PORTION OF THE UTENSILS.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Repeat Observed wooden shelving above the 3 compartment sink. Observed cardboard beneath the dough prep shelving. Surfaces shall be painted, stained, or otherwise waterproof to prevent any sources of bacterial growth.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Repeat Observed a cutting board which had been screwed into the top of the chest freezer. Food contact equipment must be able to be broken down and properly cleaned underneath.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Observed that the facility had containers holding food that were not foodgrade including dough in trash bags.

3717-1-05(H) / Capacity.
Observed that the womens restroom did not have any hot water. Ensure that all sinks are properly stocked with soap, warm water, and paper towels.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Observed that the facility had a large amount of unnecessary items around the facility. Including inside old unused equipment, extra equipment, drills and other items on shelves above the prep table. Remove items that are not necessary for the day to day operations at the facility.

3717-1-06.1(A) / Floors, walls, and ceilings.
Observed stained ceiling tiles. Ensure that there are no leaks to prevent damage to equipment or foods.
