3717-1-07(B) / Working containers - common name.
Critical Corrected During Inspection Observed working containers of poisonous or toxic chemicals that were not properly labeled with the name of the material. Bottles of cleaner were not labeled and out of original container.
Working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.
3717-1-07.1(B) / Restriction.
Critical Observed unnecessary chemicals being stored on the premises. Noticed some insecticide in the office, used for bees outside during the summer. Please keep locked up in an office cabinet to restrict access.
Only those poisonous or toxic materials that are required for the operation and maintenance of a FSO or RFE shall be allowed.

3717-1-04.3(A) / Equipment, clothes washers and dryers, and storage cabinets location
Repeat Equipment, clothes washer and dryer, and/or storage cabinets were placed in an unapproved location. Washer/dryer and clean linen items are stored in the toilet room. Washing machine was moved to this location during the most recent updates. The plan may be to move it to the office when the shelves in the office are removed.
A cabinet used for the storage of food, or that is used to store cleaned and sanitized equipment, utensils, laundered linens, or single-service or single-use articles may not be located in areas described in this rule. If a mechanical clothes washer or dryer is provided, it shall be located so that the washer or dryer is protected from contamination and where there is no exposed food, clean equipment, utensils, linens or unwrapped single-service or single-use articles.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. Components of the pizza prep cooler are not in working order. Facility is aware and working on replacement.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(Q) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Corrected During Inspection Utensils, thermometers, and/or pressure guages were not kept in good repair or calibration. Several spatulas appeared dirty and not in good repair. These were discarded during inspection.
(1) Utensils shall be maintained in a state of good repair or shall be discarded; (2) Food temperature measuring devices shall be calibrated as necessary to esnure their accuracy; (3) Ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use.

Equipment components are not intact, tight or properly adjusted. Back plate on pizza cutting table is gapped and does not provide a tight seal. Food debris is accumulating in the opening.

Equipment food-contact surfaces or utensils are dirty. Pizza cutting board, can opener, and pizza oven were dirty.

3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. Floors, walls, and ceilings have build-up of oil residue.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-05.1(S)(2)/Plumbing system - maintained in good repair.
The plumbing system was not properly maintained. Sink in bathroom is leaking.

3717-1-06.1(C)/Floor and wall junctures - coved, and enclosed or sealed.
The floor and wall junctures are not properly coved or closed, or floor drains were not provided. Portions of the cove molding is detaching from the wall and do not provide proper seal.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.
3717-1-06.4(D)/Cleaning ventilation systems, nuisance and discharge prohibition.
The ventilation system was not being properly cleaned or was creating a public health hazard. There is an accumulation of dust on ventilation system.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge. Correct By: 18-Feb-2014
