[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed bag of chicken opened and not covered in the walk-in cooler. The bag also did not have a date mark tag on it. Please ensure that food is stored in a such a manner so that it is not subject to sources of contamination. Employee weighed quantity of chicken and then threw away chicken. Corrected.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of food debris and grime on the handles of all cabinets and cooler throughout the front service area of the facility, along with at the ICEE machine stand. These areas shall be cleaned at a frequency necessary to prevent the accumulation and build-up of this food debris and grime. Employees were in process of cleaning these areas during inspection.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

Observed proper use of gloves and handwashing at time of inspection. Level II ODH certificate displayed - good. Tomatoes were at 47F at 2:00pm, but were just sliced and prepared at 11am (indicated by date mark sticker) still within the cooling time frame - good. Temps are record in a log book, continue to keep record of these temps. Recommend adding the time the temperature was taken to this log. Thanks for your cooperation.
