[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed pipes from the drink station by the drive thru window and the dining area drink station that had outlets that are below the rim the of the drain under the drive thru drink station. There shall be an air gap of at least one inch to prevent any possible contamination from sewage back ups or bacterial growth. Discussed with PIC.

3717-1-04.8(A) / Equipment and utensils - air-drying required.
Observed clean pans on the rack that were stacked without properly air drying first. Equipment and utensils must be allowed to completely air dry before stacking/storing.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed a significant build up of ice on the ceiling of the walk in freezer and in the small freezer used for french fries. Ice shall be removed or defrosted to protect food from any damage caused by ice build up.

Received complaint from a male customer that he ate a whopper with lettuce, tomato, and onion at 2 pm on Friday from the drive thru window and then became ill at 2 am on Saturday morning with vomiting and diarrhea, which lasted through to mid day Monday the 18th. Discussed complaint with PIC(person in charge), Whitney. She was aware of the complaint. I observed the employees washing their hands and changing gloves when required. According to PIC no one has been sick or called off lately. Observed the facility's employee health policy which addresses all the symptoms and food borne diseases that employees must report to management. Observed all TCS foods marked with a time sticker to indicate the time by which it must be thrown out. Facility is properly discarding all foods once they pass the time on the time sticker (4 hour hold time for everything). All coolers and hot holding units were holding appropriate temperature. Quaternary sanitizer is at the appropriate concentration.
