[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Observed several souffle cups of sour cream that were not stored properly dated per the owner. Any ready to eat TCS food held longer than 24 hours must be properly date marked to prevent them from exceeding 7 days.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.
3717-1-06.4(K) / Controlling pests.
Critical Observed mouse droppings in the kitchen behind the prep table and underneath some of the prep tables in the corner. Pest population shall be adequately controlled to prevent any contamination of foods.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions.
3717-1-07.2 / Poisonous or toxic materials: storage and display - separation
Critical Corrected During Inspection Observed that the facility had strong caustic chemicals hanging in the bar hand washing sink. At the time of inspection these bottles were set beneath the sink. DO NOT STORE CHEMICALS WHERE THEY MAY CONTAMINATE FOODS OR FOOD CONTACT SURFACES.
To prevent contamination, poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, single-service articles, or single use articles by separating the poisonous or toxic materials, and locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles, or single-use articles.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Observed an ice bin at the bar that was not installed as a permanent fixture. Properly bracket/seal the ice bin and properly plumb it to a floor drain.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Observed that the hand washing sink in the kitchen was damaged and leaking. A plumber visited the facility during the inspection to repair the sink. Ensure that the handwashing sink is properly stocked and working at all times to prevent a build up of debris.

3717-1-06.1(A) / Floors, walls, and ceilings.
Observed that the floor of the kitchen had a areas where the paint was coming up. Repaint/ repair the floor so that it is smooth and cleanable. Per the owner the facility has the floors lined up to be replaced.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.2(J) / Ventilation - mechanical.
Observed that the facility had two electrical fryers without proper ventilation. Equipment that produces grease-laden waste must have a properly installed fume hood per the Ohio Building Code. The inspector forwarded this to the township fire department.

Observed open fryers that were in use in the kitchen that did not have a proper fume hood prepared. This information was sent to the fire department.

Observed that the facility had its women's restroom closed off because the toilet was being replaced. The single restroom at the facility did not have a covered lidded trash can. Restrooms used by women shall have a lidded trash can to properly dispose of hygiene products.
