[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Corrected During Inspection Observed terijaki chicken and raost beef to be holding at 45F in the two door cooler in the prep area. All TCS foods that are being held could shall be at 41F or below in order to prevent the growth of harmful organisms that cause foodborne illness. Employee moved all of the TCS food from this cooler into the cooler in the front that was holding colder than the two door cooler. Corrected.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less.
3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Observed an accumulation of seeds in the handwashing sink behind the service line in the subway portion of the facility. Please ensure that handwashing sinks are only used for handwashing in order to properly wash hands and prevent foodborne illness.
To prevent contamination, a handwashing sink may not be used for purposes other than handwashing.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility was unable to provide proof of completion of the Level II Food Safety course. Please ensure that the ODH certificate is readily available at the time of next inspection. Feel free to e-mail a copy of this certificate to our office in the mean time if obtained before next inspection.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed two door cooler in the prep area to be holding at 44F, causing the TCS foods to be out of temperature. Please ensure that this cooler is serviced and holding at a proper tempertuare to ensure that TCS foods are held at proper temperatures to prevent growth of harmful organisms.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed walk in freezer to have a large accumulation and build up of ice in the back left corner. Please ensure that the freezer is functioning properly to prevent this ice build up from occuring.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed the Pepsi cooler and the Coca-Cola cooler to have pop stains and residue build up on the bottom of the cooler under the shelves. Please ensure that these coolers are cleaned at a frequency necessary to prevent the accumulation of food debris.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Observed the dump sink in the self-serve fountain area to have a bucket underneath the pipes in the cabinet collecting water. The leak has caused the cabinet to show evidence of water damage. Please ensure that all plumbing in the facility is maintained and kept in good repair.
All plumbing should be maintained in a state of good repair as per the Ohio Building Code.
3717-1-06.1(M) / Outer openings - protected.
Observed the back door to have sever rusting at the bottom that has caused a hold to be made at the bottom of the door. Observed the peep hole in the door to be completely open with no lense. Please ensure that all outer openings are covered and protected against the entry of pests and other sources of contamination.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and food debris in the self-serve fountain area. The cabinets were stained with old spills, the trash cabinets had trash accumulating around the can, and coffe beans and grounds were found behind the coffee machines along with a layer of dust. Please ensure that this area is cleaned at a frequency necessary to prevent the build-up of dirt and food debris.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(A) / Repairing.
Repeat Observed several ceiling tiles to have evidence of water damage throughout the facility, primarily in the retail section. Observed cracked and broken floor tiles at the 3 comp sink and underneath the two door coolr in the prep area. A work order for these tiles has been placed, facility is waiting on management to come in an retile the entire area. Observed cracked and water damaged blocks around the mop sink.
The physical facilities shall be maintained in good repair.

Gloves, thermometer and test strips available at time of inspection. Observed proper use of gloves at time of inspection. Please ensure that the entire facility and all of the equpiment is cleaned at a frequency necessary to prevent the build-up of dirt and food debris. Continue to make improvements on the repairs in the facility.
