[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Observed that the facility had cut tomatoes and cut lettuce that was not properly dated. At the time of inspection the PIC properly dated these foods. Cut melons and leafy greens require date marking.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.

3717-1-04.1(Y) / Temperature measuring devices.
Observed that the thermometer in the prep cooler is damaged. Replace the thermometer so that the temperature is readily available for employees.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed some grease on the shelves around the fryers. Observed handles to the prep cooler were sticky. Clean nonfood contact surfaces thoroughly and often to prevent any build up.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.1(A) / Floors, walls, and ceilings.
Observed a leak in roof with a bucket in the dining area. Observed tiles broken throughout the facility. Maintain facility in good repair free of leaks and surfaces smooth and easily cleanable.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.1(M) / Outer openings - protected.
Observed that the facility had light visible through the front doors and the rear door. There should not be light visible around or under doors. Purchase weather strips/sweeps for the doors to prevent insects from entering the facility.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
