Critical Food contact surfaces not easily cleanable. Observed cracked lids and a plastic measuring pitcher that is melted. Discard all damaged equipment as it is no longer smooth and easily cleanable

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Repeat Re-use of single-service or single-use articles. Observed single use containers such as sour cream containers, vinegar bottles, etc. being re-used for food storage. Discard such containers once they are empty of their original contents, as these containers are not designed to be washed and re-used.

Repeat Equipment not approved by a recognized testing agency. Observed home-use food storage container used to hold desserts in the chest freezer. Remove home-use container as it is not approved for commercial use by a recognized testing agency.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed tape on the outside of food storage containers that had been washed. Remove tape from containers as it could harbor bacteria/build-up
