Critical Person in charge did not ensure their employees are properly trained in food safety.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed pans of cooling chicken wings stored on top of other pans of chicken wings without a barrier to protect the wings from the bottom of the pan placed on top of it.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed an overfilled pan of ham with the top layers holding at 51 degrees. The ham was discarded until the top layer was holding at 41 degrees. Cease overfilling pans past the fill lines.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed pans of cooked chicken with the date marking stickers sweating off of them, making them unable to be read.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Handwashing sink water below 100°F. Observed no hot water in the women's restroom handwashing sink.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Improper storage of food items. Observed a bag of onions stored on the floor in the walk in cooler.

3717-1-02.3(C) / Hair restraints - effectiveness.
Repeat Corrected During Inspection Food employee(s) not wearing a hair restraint. Observed an employee working with food without a hat on. (C) Hair restraints - effectiveness. Food employees shall effectively restrain hair by wearing hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food; clean equipment, utensils, or linens; or unwrapped single-service or single-use articles. This provision does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands during food preparation. Observed an employee prepping dough while wearing a watch and dangling bracelets. (G) Jewelry - prohibition. (1) Except as provided in paragraphs (G)(2) and (G)(3) of this rule, while preparing food, food employees may not wear jewelry on their arms or hands. (2) A medical alert bracelet is permitted when a reasonable accommodation is made, such as wearing the bracelet high on the arm or secured in a manner that does not pose a risk to the food but provides emergency medical information if it is needed. (3) A plain ring such as a wedding band may be worn.
