3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. PIC was unable to provide proof that employees are informed of their responsibility to report illness. Provide documnetation that employees are informed of their responsibility.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Handwashing sink not accessible. Both hand sinks (in the kiosk and back room) were not easily accessible as there was lots of stuff cluttered around it. The hand sinks shall be easily accessible to promote proper adn frequent hand washing. Remove the items from around the hand sink.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical Corrected During Inspection TCS foods not being hot held at the proper temperature.The mini pretzel hot dogs were holding at 122F. All TCS food shall be held at 135F or greater to prevent the growth of bacteria. Discard the out of temperature food.

3717-1-04.4(I) / Manual warewashing equipment - wash solution temperature.
Critical Manual wash solution not being maintained at 110˚ F or above. The hot water at the 3-compartment sink never got more than 69F. The wash solution temperature shall be maintained at 110F to properly clean dishware. Restore the hot water to at least 110F to wash dishware.

3717-1-05(H) / Capacity.
Critical Water source and/or hot water capacity not sufficient. There is no hot water available in the back storage room where warewashing is done. The PIC stated that the hot water tank has not worked for at least six months and they have been filling jugs of hot water from the Bunn hot water beverage dispenser to take back to the store room to wash the dishes. Hot water is necessary for all hours of business. Repair or replace the hot water tank by Monday, May 23, 2022 or the facility will be closed.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Handwashing sink water below 100°F. The water temperature at the hand washing sink in the back room was only 69F. All hand sinks shall have at least 100F water for hand washing. Restore the hot water.

3717-1-04.6(C) / Hot water and chemical sanitizing - methods.
Food-contact surfaces and utensils not sanitized properly. The quat sanitizer in the wipe cloth bucket was low at time of inspection. The quat sanitizer dispensing from the 3-compartment sink also tested low, however, the water temperature was below the minimum 75F required titest properly.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Working food containers not properly labeled. Provide labels forf the bulk food bins.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Observed boxes of single-use food cups stored directly on the floor in the back room. Store all single-use items at least six inches off the floor.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Observed single-use cups being used to store salt and as a scoop in the bulk food bin. Provide reuseable containers/scoops.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the cabinet door is off its hinge at the dipping sink. Repair the door.

3701-21-25(I) / Person in charge certification in food protection
FSO did not have a person in charge per shift with the person in charge certification in food protection.

The facility has no hot water in the back room. The hot water tank shall be repaired/replaced by Monday, May 23, 2022 or the facility will be closed to make the necessary repairs. A reinspection will occur on Monday to verify.
