Critical Equipment food-contact surfaces or utensils are unclean. Observed build up on the interior drop plate of the ice machine.
Clean the ice machine interior as often as needed to maintain it clean to sight and touch.

3717-1-02.3(C) / Hair restraints - effectiveness.
Repeat Food employee(s) not wearing a hair restraint.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed torn door gaskets on one of the reach in refrigerators and one of the reach in freezers in the back room.

3717-1-06.3 (C)(2) / Employee accommodations - designated areas: lockers
Observed an employee beverage and cell phone stored on the counter at the front with/above food and food contact items. All employee belongings must be stored in a designated area that is separate from or below all food and food contact items.

3701-21-25(I) / Person in charge certification in food protection
Repeat FSO did not have a person in charge per shift with the person in charge certification in food protection.
